AIMVT CHECKLIST

√ Photocopy of Diploma from an accredited veterinary technology program or equivalent and/or Credential to practice as a veterinary technician by a state.
√ Completed Waiver, Release and Indemnity Agreement Form.
√ Completed Professional History and Employment History Form: 3 years with 6,000 hours of experience, completed within 5 years of application, 75% of time in field of internal medicine in the subspecialty of choice, minimum.
√ Proof of Attendance and outline of continuing education courses attended specific to the applicant’s subspecialty submitted on the Internal Medicine Continuing Education Form - 40 hours minimum.
√ Completed Advanced Veterinary Internal Medicine Technician Specialty Skills Form in area of specialization.
√ Completed Advanced Veterinary Internal Medicine Technician Specialty Knowledge List Sign off sheet.
√ Case Log Report Form – a minimum of 50 cases (maximum of 75) typed on the form. All cases must be from October 1st of the preceding year through September 31st of the year of application.
√ Four In-Depth Case Reports selected from the Case Log Report.
√ Three Potential Examination Questions specific to the area of specialty.
√ Two letters of recommendation. (Sent separately by the letter writers)
√ One complete application packet, in the above order, in a 1-inch, 3-ring binder. Three additional photocopy sets, each set stapled, of the case logs and case reports only. One CD-R with the case log and all (4) complete case report files. The CD-R should be labeled with your name.
√ Application fee. Check in the amount of $50 dollars made payable to AIMVT.
